Make your policies and records searchable without pasting PHI into a public chatbot.

Healthcare teams sit on thousands of pages of policies, procedures, contracts, and records that nobody can actually search. The fix is not a consumer AI tool. It is a retrieval system deployed inside your own boundary that answers from your own documents, cites its sources, and never ships protected health information to a service you have no agreement with.

The information exists. Nobody can find it.

Clinics, senior living operators, and healthcare back offices share the same three problems, and the workaround staff reach for is the one that creates a breach.

Answers buried in PDFs and binders

Admission criteria, medication policies, payer rules, and survey-readiness procedures live across hundreds of documents. Staff either dig for twenty minutes or ask a colleague who might remember, and both paths are slower than the work they interrupt.

Staff improvise with public AI tools

When search fails, people paste text into whatever chatbot is handy. If that text contains protected health information and the vendor has no Business Associate Agreement, that is an impermissible disclosure: a reportable compliance problem, not a productivity win.

Every answer depends on who you ask

Without a system that returns the actual policy text, answers drift. Two staff members give two versions of the same rule, and nobody can point to the paragraph that settles it until a surveyor does.

How retrieval-augmented search works

RAG, retrieval-augmented generation, is a plain idea: search your own documents first, answer only from what was found, and cite the source. The engineering is in the pipeline around it.

  1. Ingest and index your documents

Policies, procedures, contracts, and records are collected, OCRed where needed, split into sections, tagged with metadata, and indexed for retrieval by meaning, not just keywords.

    • Inputs: PDFs, Word files, scans, and exports from the systems you already run.
    • Discipline: document types, effective dates, and ownership are captured up front so stale versions do not pollute answers.
  2. Retrieve and answer with citations

A question pulls the most relevant passages from the index. The answer is generated only from those passages, and every claim links back to the file and section it came from.

    • Guardrail: if the documents do not contain the answer, the system says so instead of inventing one.
    • Output: the answer plus the exact source text, so staff can verify in one click.
  3. Govern, review, and improve

Access control decides who can search which document sets. Logs record what was asked and answered. Low-confidence answers route to human review, and corrections feed back into the index.

    • Controls: permissions, audit logs, retention rules, and review queues.
    • Adjacent problem: data trapped in scanned paper is its own pipeline. See OCR to SQL.

Deployment paths that keep PHI where it belongs

The model is just a component. The compliance posture comes from where the system runs and which agreements cover it. Our HIPAA environments page covers the full operating model.

Local, air-gapped deployment

For the strictest environments, the documents, the search index, and the models all run inside your network. Nothing about the workflow depends on an outside service ever seeing your data, and the update and logging paths are designed local-first.

AWS with HIPAA-eligible services

When cloud is acceptable, we build on AWS HIPAA-eligible services with encryption at rest and in transit, role-based access control, audit logging, and Business Associate Agreements aligned to how the data actually moves.

Built by a team that operates HIPAA platforms in production

These are first-party systems we build and run in HIPAA-sensitive healthcare environments today.

Dexzyle and Senior Insight

Healthcare communication and EHR/eMAR.

Dexzyle is our healthcare communication platform with fax and video calling live in production. Senior Insight (seniorinsight.com) is an EHR and eMAR platform for senior living communities where we support cloud infrastructure, data systems, and HIPAA-compliant integrations for resident care and medication management.

seniorinsight.com

Taliswitch

Secure care-team workflow.

A secure healthcare communication and workflow platform for senior living and post-acute care teams: secure messaging, prescription visibility, document context, and audit trails across connected EHR, pharmacy, and document systems.

taliswitch.com

The same team runs the automated document pipeline behind OilGasOrGrass.com: daily ingestion and OCR of scanned regulatory filings, dual-model AI extraction, and scheduled warehouse loads. Document search is the retrieval half of that same discipline.

What a first project looks like

Incremental on purpose, so you can judge answer quality on your own documents before anything touches production workflows.

  1. Discovery conversation

We talk through the documents, the users, the questions they actually ask, and where the PHI boundary sits. You leave with a clear picture of scope and deployment options, with no commitment required.

  2. Scoped pilot

A pilot on a real document set, such as your policy manual, a records subset, or a contract library, so your team can test answer quality and citations against documents they know cold.

  3. Production hardening

Access control, audit logging, review queues, retention rules, and an operational runbook. The pilot becomes a system your team relies on and your compliance officer can defend.

Questions healthcare teams actually ask

Is it a HIPAA violation to paste patient information into ChatGPT?

If the text contains protected health information and the tool is not covered by a Business Associate Agreement, yes — that is an impermissible disclosure. The safer path is a system deployed inside your own boundary, covered by the right agreements, that answers only from your own documents.

What is RAG in plain terms?

Retrieval-augmented generation means the system first searches your own documents, then writes an answer using only what it found, with citations back to the source files. It is closer to a very good search engine than to a chatbot with opinions.

Can this run without sending data to the internet?

Yes. For the most sensitive environments we design local, air-gapped deployments where the documents, the search index, and the models all stay inside your network. When cloud is acceptable, we build on AWS HIPAA-eligible services under a Business Associate Agreement.

Have you actually built HIPAA systems, or is this theoretical?

We build and operate HIPAA-compliant healthcare platforms in production today, including Dexzyle, Taliswitch, and the Senior Insight EHR and eMAR platform at seniorinsight.com. Document search builds on the same engineering discipline those systems run on.

How does a project like this usually start?

With a conversation about the documents, the users, and the boundary — followed by a scoped pilot on a real document set so you can judge answer quality before anything touches production workflows.

Talk through your document problem

Tell us what your team cannot find, where the documents live, and how strict the boundary needs to be. We will tell you honestly whether a retrieval system is the right fix.

  • We reply by email with an honest answer on whether a pipeline is the right fix.
  • If it is, the next step is a scoped pilot on your real documents.
  • Denver, Colorado. We sign Business Associate Agreements where PHI is involved.
